Forms
If you would like to obtain a license or enter a
race
you can do so by filling out the forms linked to this page. Each of the
files are in Adobe Acrobat format (.pdf). You can download the free Acrobat
Reader from Adobe.com. Once you are viewing the forms all you have
to do is print it and fill it out.
All
forms which are filled out for the purpose of
entering
a minor must be notarized when mailed in, and
accompanied
by the minor waver forms.
You must fill out both parts of the minor waver
form for it to be valid.
